Geez. I am almost to the point of never eating at a restaraunt that doesn't have an open kitchen.
A few years ago a friend went to eat at a local Itallian place. There was one of those loud crash bang booms and everyone turned to see what had happened, but it was obvious that someone had toppled a tray of glasses.

When his dinner came, he was a brought a new glass of water (they don't refill). Afetr finishing and washing down the marinara soaked linguini bits with the water, he began the after dinner converstaion and tilted the cup to his mouth. What he thought was a frozen water cube fell into his mouth and he began heartily crunching on an ice cold, chunky piece of glass.

: Years ago I worked in Woolco (sister to Woolworths, precessor of WalMart), and was sent into restarant to get cleaner when someone broke something in my department. The cook was smoking while he worked, and dropped lettuce destined for a burger on the floor. He picked it up, wiped it in his pant leg, slapped it in the burger and sent the order down the line. I reported him and he was fired that same day. Unfortunately, he was a favorite and long standing employee, and my job was never quite the same after.
